Insurance & Fees

With the following info:

Insurance Accepted: Blue Cross Blue Shield PPO

Will also accept private pay and can provide a form for possible out of network reimbursement.

Therapy Session Fees:

     -Kristy Johnstone, LCSW, MSW  $155 per clinical hour

     -Mia Benker, LSW, MSW, Dionna Mantucca, LSW, MSW, Justyna Barzykowska, LSW, MSW  $135per clinical hour                    

Full session fee cost will apply if not covered by your Blue Cross Blue Shield PPO insurance plan.  It is your responsibility to know your coverage.
